Everything you need to know before your first appointment. Use this checklist to prepare for a smooth, stress-free start to your care at RECO Integrated Psychiatry.

What to Bring

Being prepared for your first appointment helps us make the most of our time together. Please bring the following items:

Valid photo ID

Driver's license, state ID, or passport

Insurance card(s)

Front and back of your primary and any secondary insurance cards

Current medication list

All medications you currently take, including dosages and frequency (prescription, over-the-counter, and supplements)

Previous medical records

Any psychiatric evaluations, therapy notes, or hospital discharge summaries (if available)

Referral letter (if applicable)

If your insurance requires a referral from your primary care physician

Payment method

Credit card or debit card for any copays or coinsurance

Your First Appointment

Your initial psychiatric evaluation typically lasts 60 to 90 minutes. During this appointment, your psychiatrist will:

  • Review your complete medical and psychiatric history
  • Discuss your current symptoms, concerns, and treatment goals
  • Perform a comprehensive diagnostic assessment
  • Discuss potential treatment options and recommendations
  • Answer any questions you or your family may have
  • Create an individualized treatment plan

Please arrive 15 minutes early to complete any remaining intake paperwork. If you are completing forms online ahead of time, you will receive a link via email after scheduling.
