That is why every patient at our practice receives a personalized aftercare plan designed to maintain progress, prevent relapse, and support continued growth.

Your aftercare plan may include ongoing medication management, step-down therapy sessions, wellness recommendations, and connections to community resources. Our team works with you to create a sustainable roadmap for your continued well-being.

As part of the RECO Health Network, we can also coordinate transitions to other levels of care within the network when needed, ensuring you always have the right support at the right time.
